CONSENT TO PROCESS PERSONAL DATA
According to Article 6 Paragraph 1 Letter a) and Article 7 of the Regulation of the European Parliament and the Council (EU) 2016/679 of 27 April 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (“General Data Protection Regulation“)

A. The data subject consents to the processing of his or her personal data to the following extent:

1. For the purpose of negotiations on the provision of medical or other suitable services and measures

The data will be processed for the period of 3 months since the day of the consent unless a medical services contract has been entered into

2. For the purpose of sending information, offers of other suitable services, goods and communication
a) title, first name, last name, address, email, telephone number, year of birth, tel. conversation
b) the type of ophthalmic error or other medical conditions, the type of healthcare procedures
The data will be processed for 10 years since the day of the consent

3. For the purpose of providing a credential on the procedures and services provided
a) title, first name, last name, address, email, telephone number, year of birth, tel. conversation
b) the type of ophthalmic error or other medical conditions, the type of healthcare procedures
The data will be processed for 10 years since the day of the consent

4. For the purpose of the website proper functioning, measuring the usage level, and promotion services when utilizing the web presentation of the company
a) technical cookies necessary for the proper functioning of the website and on-line services
b) cookies used for the purpose of measuring of the usage level, and of generating statistics regarding the usage level and behavior of the website visitors (Google Analytics services)
c) promotional cookies of Google Inc. and Seznam.cz (processors) in the Google Adwords and Sklik systems
The data will be processed for 10 years since the day of the consent. The functioning and compatibility of the website may be limited during inactive cookies

5. For the purpose of the protection of your safety and property
a) CCTV video recording
b) Sound recording
The data will be processed for 14 days since the day of the recording

6. In order to analyze the data from the examination and anonymous publishing of the data for medical and scientific purposes
a) Year of birth, health condition, occupation
b) Type of eye error, results of measurement, findings and clinical outcomes, performed medical interventions

7. For the purpose of providing follow-up care, receiving personal data from secondary (post-operative) care providers
a) Name, surname, year of birth, state of health, occupation
b) Type of eye disorder or defect, measurement results, findings and clinical findings, treatment and interventions performed

B. The following entities (controllers) take the responsibility for the processing of data:

C. The above-given data may be provided to the following entities for the purpose of their further processing (processors):
– External suppliers of the information system, External suppliers of the technical support

D. Should some of the provided medical or other services and procedures be realized through a subsidiary or economically connected companies as specialized entities, the above-given personal data may be also provided to such entity

E. According to the General Data Protection Regulation, You, as a subject whose data are processed, have the following rights

a) the right to be informed on what personal data are processed
b) the right of the access to such data, the right of update and rectification of personal data
c) the right to processed data portability to another controller
d) the right to restriction in processing or erasure of the data
e) the right to lodge a complaint with a supervisory authority, and the right for the judicial protection should You assume that Your rights according to the Regulation have been infringed due to the processing of Your personal data in breach of the Regulation

F. The provided consent may be withdrawn at any time in the following manner:
a) via an email sent to the email address of the contact person
b) via a written communication sent to the address of the controller
